Scheduling Well Visits
TIPS:
We encourage you to choose a physician or nurse practitioner as your primary provider. You can schedule all of your well checkups with him or her.
Bring your child’s immunization record with you. Bring any camp or sports physical forms with your child’s information portion already filled out.
Don’t promise your child that he/she will not be getting any shots! We have many new immunizations that are now recommended for many different age groups.
Prepare your child to have to get undressed and wear one of our stylish paper gowns.
Remember to ask for school medication forms for your children if they have chronic illness and you expect the school nurse to administer medication.
We try to keep the office running as smoothly as possible, but emergencies and late arrivals may delay our normal schedule. Please realize that we always try to give each patient our full attention. Since your child may need extra time at some point in the future, please be understanding when there is an unexpected delay.
Routine visits are scheduled as follows with the recommended immunizations, tests and procedures. At times, however, we may vary from this schedule.
Our practice follows the immunization guidelines recommended by the American Academy of Pediatrics.
